People were really creative at the time in making maximum
use of the technology available to them.  Teletypes had the
ability to underline and embolden by overstriking, and this was
indeed used not merely for highlighting but as actual part of
the syntax when entering code.  For example, some dialects of
Algol allowed stropping by underlining, and Mirfac (because
it was mentioned previously on this list) allowed the use of
"red ribbon" for "comments and also for those functions whose
names are combinations of English letters".

(In printed matter, such as in academic journals, Algol
traditionally used boldface for highlighting keywords.)
