Indeed. So default would be 4 for footnotes in a separate element
(with a header) and at the end of the page?
Maybe even better would be to have
FOOTNOTE_END_HEADER_LEVEL
and
FOOTNOTE_SEPARATE_HEADER_LEVEL
for the two cases?
