h1 output when footnotestyle is separate.
Surely footnotes should never be introduced by an <h1> -- they are
subsidiary by definition. Maybe they should be one less h-level than
the current sectioning level?
footnotes-heading about-heading contents-heading shortcontents-heading
Sounds just fine to me.
