Yes, that is one of the capabilities of GNU Health. Each department can work on 
an independent node (GNU Health HMIS ) or share one, e.g. within a hospital or 
primary care station. The nodes can connect to the federation server, which can 
be a central, nationwide instance. Check out Luis Falcons presentation at oSC
