This main motivation for putting it into one file is to concentrate
on the mechanism that makes these functions work for compilers that
don't know how to inline.
if you organize things based on implementation, when the implementation
changes, the organization must change, too. this is latent drudgery...
long term, it's easier to organize things based on interface, pour
incidental implementation into those containers, and slosh them around
later if need be.
