Follow-up Comment #21, patch #7559 (project avrdude):

If someone hooked up an AVR to a real RS-232 interface, the +-12V would
probably blow it up.  So I think the code is fine.

You're right, though, that the comment in arduino.c is confusing and, I think,
wrong.  It's probably worth submitting / applying a patch for that.
