First is the above ok?
Yes, it looks reasonable to me.
Second, should TOP_NODE_FILE be set in the default case?
I don't have strong feelings; is there something plausible to set it to?
The defaults as you gave them are a bit unusual, but not unreasonable,
seems to me.
Looking at your list, the only thing that came to mind was that there
should be a warning when --split is specified but the output isn't in
fact split, e.g., your first case:
./texi2any.pl --plaintext --split=chapter manual.texi
=> output on stdout, not split.
