accessing NoteHead from a merged note

Is there a way to access one NoteHead grob from another
NoteHead grob that it is merged with?  Like this:

    \once \override NoteHead.after-line-breaking =
    #(lambda (grob)
       ; how to access the NoteHead grob of the
       ; c'' in the other voice from here?
       #f) 
    c''4
  }
  \\  
  { <e' g' c''>4 }
