It's not "Oops", it's the development methodology.

Under a normal methodology the developers should:

1) diagnose a bug;
2) find what they think is a fix;
3) rebuild the whole thing;
4) make sure the bug is fixed and to the extent possible than no new bugs have 
been introduced;
5) _finally_ release the fixed code to public.
