That's possible, I suppose: that version is over six years old and I
can't recall whether these were indexed properly back then or not.

No, sorry, those line numbers are not the lines in the index where the
reference to + appears.  They are the lines into the topic ("MAKE
Variable", etc.) where the reference appears.
