The value of $prefix is bogus. It points to ESP (EFI System Partition)
but with non-existing path. One of (hd0,gpt1)/EFI/ubuntu or direct LVM
reference would work.

Quick workaround is to create /boot/efi/boot/grub and copy grub.cfg
from /boot/efi/EFI/ubuntu in there.

Does it work?
