VGA terminal fixes

Hi,

The VGA terminal has some problems with scrolling:

1) There is an off by one bug that makes it crash.

2) The map to read from is not selected.

3) When 1+2 are fixed, the scrolling produces garbage on the screen
   while scrolling.

Here is a patch to fix all these issues. #3 is solved by using double
buffering. At the moment that seems the best solution to me. To make
this possible two pages are required, so I changed the mode from mode
0x12 to mode 0x10. So now the screens height is 350 instead of 480.

After this patch is committed I will start working on making the
framebuffer more generic.

I assume this patch is ok and I will commit it next Tuesday.

Thanks,
Marco
