[Groff] PDFmarks hacks for MM macros

Here's some macros you can source that will add outline ("boomark")
PDFmarks to an MM macros document.

The outline will have levels for each heading level, and it should
work no matter how your re-arrange the Postscript after groff has had
its way with it.
