The reason is that you have notes straddling barlines, so LilyPond
cannot find a place to break the music. In bar 2 you have 1/8 note short
in duration so the 1/2 note that follows is actually straddling the
barline and so LilyPond cannot break a line there, this tends then to
continue with successive bars having notes straddling the barlines,
which can lead, in extreme cases, to the music running off the end of
the line.
Use Score->Check Score to look for errors.
