My understanding is that blip.tv lets you do most of this. Unlike many
of these web video sites, Blip provides a link to the original format in
addition to re-encoding the website in Flash. From what I can tell of
their blog, they support using Theora as the original. I don't know if
they can convert from another format to Theora

I've not used the site myself to try and do this though so if my
understanding based on what I've read is incorrect, someone with more
first hand experience should correct me.
