Ok, I downloaded the new cvs (0.5.0).  Success for the most part.
Here's the steps:

jpeg2swf -r 10 -q 100 logo.jpg logo.swf

script.sc:
.flash name="newlogo.swf" fps=10
  .swf logo "logo.swf"
  .frame 0
  .put logo x=2 y=333
  .frame 481
.end

# movie.swf was recorded at 10 fps
swfcombine -Tm movie.swf newlogo.swf -o newmovie.swf

Here's the result:

The logo appears for the entire length of the movie but there's some
funky graphics delay with everything else.
