The PDF version from the CVS sources also seems to be OK when I view
it with xpdf, gv, or acroread.  I don't remember any recent changes,
so I would expect it to be OK in the 2.1.57 distribution as well.

The refcard-letter.pdf is being produced by pdftex from *.tex
file. The resulting papersize is 8.5"x8.5" (or 612 x 612 pts):

If I rebuild pdf file from postscript file using epstopdf I get
a (correct) 8.5"x11" (612 x 792 pts) papersize:
