I have been having the same problem since commit e8d36bf10.  Commit
a6eea7b4d did not fix it.  The symptoms are only noticeable when I ssh
into my box from the outside.  It doesn't appear to matter whether
I use PuTTY or rxvt (in both cases $TERM==xterm).

When I edit a long file in Vim and scroll the window, I see the cursor
jumping all over the screen.  The speed of the scroll is much
diminished.  Again, I am only able do discern this effect when
connecting to my box via ssh.  When I attempt to reproduce from
a local terminal, I am not able to notice any slow performance.  And
when I run a build of screen pre-e8d36bf10, I see no performance
impact, local or remote.
