Yes

It's the ANSI code for up arrow.  I might have hit it once or twice, but I
didn't hit it hundreds of times.

running in screen at time it happened? 

I don't remember exactly what I was doing.  I spend most of my time in bash
and emacs.

iTerm 2

>> Can you provide output of `env`?

relevant.
