I understand that this is optional, but I think that it does not fit
with most users' workflows. A "quilt refresh" is, in my workflow,
similar to "git commit" -- only done when I've made what I believe to
be a coherent set of changes. I don't want to refresh my patch every
time I exit $EDITOR to run 'make', for example, lest I accidentally
overwrite a perfectly good patch with a boneheaded edit.
