Not especially efficient, re-allocating for each line.

How about

    }
    ...
    return str;
}

    for (...) {
        qemu_hexdump_line(str, ...);
        fprintf(fp, "%s: %s\n", prefix, str->str);
    }
}

So that we reuse the one allocation across the whole loop.
