are their 2 password boxes on this login form?  if so, one puts you in
the header manager again and the other continues setup.  They are
password only with seperate buttons, using the passwords that you set
in the header manager.

the header.inc.php should be in the phpgroupware root, in the same
directory as the header.inc.php.template file.  If it's not there, or
is somehow not readable by the web server (it does not have to be
writable after it is created) then you won't get any password prompt
and will just get the header manager again.
