Please give us the versions. (apache, php, mysql/pgsql, phpgw)

Is the email on another server?

How about the db?

Don't look just for swapping, also look for long periods of high cpu usage.

Is the delay from certain modules to email or from all tested? (I just throw 
this
one in, I don't know what it would signify)

You may have figured out that this doesn't happen to others, so let's go through
your settings.
