[Phpgroupware-users] Problems installing phpgroupware on header.inc.php

Hi,

I installed phpgroupware (0.9.14.003.zip) on my NT4 Workstation with
Personal Web Server and PHP 4.0.3b
Everything worked fine but now I am setting up the header.inc.php

Here are my settings:
Server Root: 123.456.789.012
Include Root: 123.456.789.012
Admin password to header manager: mypassword
DB Host: 123.456.789.012
DB Name: phpgroupware
DB User: username
DB Password: userpassword
DB Type: MySQL
Configuration Password: mypassword
Persistent connection: True
Sessions Type: php4
Enable MCrypt: False

I pressed the >write config< button and got the message:"Created the
header.inc.php" After continueing I receive the following messages:
