[Phpgroupware-users] Trying to import contacts to addressbook

I have run into a little problem that I I am not sure of what the cure is 
because
the apache logs aren't giving any clues.  When I try to import into the
addressbook, using the ALTCSV button,  I choose a file and then hit download.  
It
simply seems like it is refreshing the screen and I get the following in my 
apache

(Doamin name and ip masked to protect the innocent)

Any Ideas??  Thanks (If there is another log I can look at to help figure this
out,  please point it out.)
