OK there is a problem with the .14 rpm missing this file.  You have two
options:

1 download template file from savannah (
http://savannah.gnu.org/cgi-bin/viewcvs/*checkout*/phpgroupware/phpgroupware
/header.inc.php.template?rev=1.35.2.5
) and save it in /path/to/phpgroupware/

2 Start over with a full install from cvs using these instructions:
# cvs -d:pserver:address@hidden:/cvsroot/phpgroupware co -r
Version-0_9_14-branch phpgroupware

# cd phpgroupware

# cvs co -r Version-0_9_14-branch core_modules

Let us know if that fixes it.
