Fixed a bug in formatted_xslt_list (cat_id(HEAD) vs id(.16))

I will try to create a 18-proposal branch in cvs in the next couple of days.  
Need to recover a little bit more first.

Included "app_header" for "noframework"

A small change to class.xslttemplates.inc.php to allow explicit parsing – as 
original intended (this is very useful for creating the data for email output.)

This means that one has to call
$GLOBALS['phpgw']->xslttpl->pp();  
after the 
$GLOBALS['phpgw']->xslttpl->set_var('phpgw',array('list' => $data));
to parse the page.

To skip the header,footer and navbar – set
$GLOBALS['phpgw_info']['flags']['noframework'] = True;
