The header is created automatically by the setup process.
The setup process is run by the phpgroupware/setup/index.php
file

Sorry to be so stupid at the beginning but I have a
problem directly with the following (extract from the
install document)
Create the phpGroupWare header file
In your browser, go to http://<host>/setup.php
You will be presented with a form to fill.
Click on the "Write" button.
This should work with no problems since the
files are owned by the
web servver user.
My question is very simple :
Where is the setup.php file
How can i manage to have it ?
Should i create it ?
