I'll recomend you to upgrade onto last release (there are lot of fixes
there).
If you (for some reason) want to stay with 1.2, there's example in
monitrc that is distributed with sources:
# check apache with pidfile /usr/local/apache/logs/httpd.pid
# start program = "/etc/rc.d/init.d/httpd start"
# stop program = "/etc/rc.d/init.d/httpd stop"
# port 80
# port 443
Complete overview of supported statements is also in monitrc or in
attached documentation.
