I'm guessing you're referring to SDL Mixer here? Does the recent release of 
mikmod (after several years) change much?

Nope. An annoying bug with Mikmod is that it overrides the volume level set by SDL_mixer if the module that's currently playing has volume changes embedded in it (mostly fade-in/fade-out). SDL_mixer upstream knows about it, but it cannot be fixed (we tried.) And the better sound quality and more supported formats of modplug make it the preferable choice.
