Yes.

Is there a way to get that behavior at runtime?

No, not currently.

Is there a pre-built binary available?

No, I don't think so.  But if you have a working installation of GCC,
you can build it yourself in a few seconds.

Shouldn't HAVE_CASE_INSENSITIVE_FS be the default on a build for Windows?
