    Move {{AvName}} into a database string that incorporates it
    
    The original MST code was obscure. Its replacement more clearly shows
    that {{MonthlyChargesPaymentFootnote}} is a single, unitary footnote,
    which incidentally may happen to be composed of multiple MST strings.
