I have used yeaphone[1] on boxes with no display or keyboard. This uses 
liblinphone and a Yealink P1K USB handset which has a keypad, small lcd and 
ringer as well as audio.

I have also heard of people using wrappers around linphonec to provide 
alternative interfaces.
