It helps if you have a correctly set up DNS with the necessary AAAA
and PTR records.

I use it very successfully over IPv6. I have had long conversations
with a friend in Australia - workstation to workstation - no NAT -
even though the round trip time is something like 375 mS.

I also have an IPv6 capable Astrisk running and if you want to try my
time service or my echo service, feel free to do so. They are:

time:  sip:address@hidden
echo: sip:address@hidden

PS The services are also available over IPv4. Replace ast6 with ast4.
