I tried Media5 with iOS 4.3.4 which is also free (plus another version with fee) and background with UDP works!!, using MiniSipserver on Windows.

And works also without any keep alive setting enabled.

But developers warn about the battery drain à http://www.media5corp.com/en/ios-multitasking-and-battery , as already mentioned here http://lists.gnu.org/archive/html/linphone-developers/2011-08/msg00052.html

The important thing is :

we need the application to stay alive and connected to your VoIP server/provider, so our developers have implemented an undocumented trick to avoid the Media5-fone from being suspended and to continue executing some application logic while it is in background. Therefore:

The application stays running in the background, it keeps the registration active with the VoIP server/provider.
When somebody calls you, the server is able to contact your device, so you can receive the call.
The downside is if the application keeps registering, that means it stays connected to the network and consuming energy: that’s why the battery will last less. The same would happen if you spent the whole day browsing the Internet.

So this trick should be investigated deeper…

From Apple's documentations, there is no trace of UDP support for voip socket at all. It used to work from IOS IOS 4.0 to 4.2, but from my understanding, it has never been officially supported by Apple.

It seams UDP background mode is no longer supported in IOS > 4.2. There is no obvious solution except using SIP TCP instead of UDP.
