In slackware 13.37 package libXv-1.0.6-i486-1.txz (similar for Slack 64)
has Xvlib.h. It is in /usr/include/X11/extensions/Xvlib.h.
Did you install that package?
Also, if you look through .../slackware/MANIFEST.bz2 you can find a
list of all the files in a complete Slackware installation, as well as
what package the file is found in.
