You can redefine what is printed at the beginning of the wavy line,
like in this example:

trillflat = \markup{ \musicglyph #"scripts.trill" \flat }

\once \override TrillSpanner #'edge-text = #(cons trillflat "" ) c1
\startTrillSpan
b \stopTrillSpan
