textscript at start of bar, and hiding skipped bars?

Is it possible to put markup text above the start of the staff? or at
the start of any measure?
Sometimes you want to have some textual instruction that is associated
to a measure, not to a single note!

I also wonder if it is possible to hide (white out) empty (skipped)
bars.
