I would strongly vote for making this a "defect" again because that's what it is from the user's perspective. And it really is a defect that has to be repaired.

But apart from that discussion about words: is it really the accidental engraver that should look after that issue? Isn't it rather the process of merging the noteheads that should take care of this? I'm not sure which engraver is doing that job, but that should have information about note heads and pitches, and if the same notehead is produced by different pitches it could refuse to merge the note heads.

BTW: the combination of flat and sharp as in

  \new Staff <<
    \new Voice \relative c' { \voiceOne fis8 g }
    \new Voice \relative c' { \voiceTwo fes8 f}
  >>
}

isn't really ambiguous anymore but still doesn't seem correct to me. So checking for different pitches should be sufficient.

In the snippet below, the upper voice plays F# G, and the lower voice plays Fnat Fnat.  However, LilyPond appears to treat the first F in the lower voice as an F# - see attached image.
