https://code.google.com/p/lilypond/issues/detail?id=4197Re: #15 some words might be backwards. This patch does shorten the measures that use KirchenPausen, and lengthens those that use black-bar rests. I put the image in comment #12 to show something with natural spacing.
There are independent parameters 'bound-padding' to give space in response to the visible glyphs, and 'space-increment' to lengthen the bar based on duration. Adjusting those should let you get what you want. I looked at some Breitkopf & Härtel parts from 1910 to set the defaults.
The source for 'rest-positioning.ly' is strange. It puts multi-measure rests in two voices. If anyone did that in a score with fewer lines than required to fit the rests, he could direct the lower-voice rest-counts to go below the staff, which resolves the collision.
