Up to seeing #12 I had never seen a mono-chord in whole notes.

The first patch fixes what we know we want fixed, and leaves LilyPond able  
to separate notes on a single line enough to indicate two voices, or  
overlap enough to fit in a chord,   <e' e' g' a' b'>  , in a way reasonably  
consistent with the image from Gould's book.

Let's take the first patch.
