Furthermore the example from Gould which you quote is actually to illustrate the _minimum_ separation in contrast to one which is not acceptable. It is not stated as a recommendation. In fact she goes on to say, "Many editions place all minim and semibreve rests on only the outside stave lines, to avoid confusion."
So I don't think your patch is an improvement: it exchanges one set of ugly examples for another, but increases the likelihood of confusion. We should leave the default positioning as it is.
