I don't understand in any way how the output is generated, but here's some  
further data.  Turning on the skyline debugging shows how the SVG backend  
fails to skyline the text with a note glyph accurately, whilst using a  
rectangular skyline if the note is not there.  Contract with how this works  
with the PDF backend.
