Tremolos (as well as percent repeats, perhaps) should be played in MIDI
output by default, as if \unfoldRepeats was set.

\unfoldRepeats could still be used for volta repeats; and tremolos could be
"folded" using a specific switch (although users may typically not want to
disable this behavior, as the discussion showed).
