http://code.google.com/p/lilypond/issues/detail?id=1412Oops, it's Timidity that skips initial rests — opening the MIDI in Rosegarden shows they're intact even with the patch as is, because there are non-note MIDI events (time, tempo, text, instrument) at the rest.

No — the way to express this is by setting the time signature after the pickup. This would work for initial grace notes too. It might be possible to make Time_signature_performer (or Midi_walker?) repeat the time signature where needed, but I think that's a different issue.
