A variation of the original problem is the following:
\version "2.16.0"

untransposed = \relative c'' {
  c4 c d d
}

transposed = \relative c'' {
  % Setting the number of repetitions to 1 fixes the problem in the
  % MIDI output.  With the number set to 2, the transposition invades
  % the "untransposed" Staff in MIDI.

  \repeat unfold 2 \transposition bes

  R1
}

\score {

    \new Staff = \untransposed

\transposition bes essentially is the same as
\set Staff.instrumentTransposition = #{ bes #}
and that is

      'symbol
      'instrumentTransposition)))

It is not clear to me why this would migrate out of the original Staff, but  
then I don't understand what's happening in lily/performer-group.cc at  
all.  No idea whether that's involved as well.
