Regarding #3: noteheads are left aligned in _different_ voices.  That has  
nothing to do with attaching stems since noteheads in different voices  
don't share stems.  Within the same voice, like
\new Voice <<g'1 d'\harmonic>>
things are centered just fine.  I am leaning toward marking this issue  
invalid.
