1.  Please read http://lilypond.org/website/bug-reports.html and follow  
it.  In particular, do not post bugs directly to the tracker.

2.  Please read ALL the notation reference section on grace notes.  In  
particular, the section on known issues and warnings.

3.  Please note that your sample code is not a tiny example.  Had it been,  
you might have seen your error.

4.  Note that your code has triplet timing in the upper voice, but not in  
the lower.  Hence the grace notes are of different lengths.
