What version of texinfo.tex are you using?  Try getting the latest from
ftp://ftp.gnu.org/gnu/texinfo.tex.  Also, try running tex directly,
instead of the texi2dvi shell script, just for simplicitly.

If it still happens, then add a line:
@loggingall
before the @math command, and send me the resulting .log file from the
tex run (it will be big).


    This is math.info, produced by makeinfo version 4.3 from math.texi.

       b_l \ldots b_h

    which is not what I expect easer.

That is the best makeinfo can do.  Sorry.
