LD_LIBRARY_PATH is not used in OSX.
it's calle DYLD_LIBRARY_PATH.

I don't think I will be able to help, as I don't have any apple stuff. However, if you do get it working, I think you may the first person using the dicom package on OS X. Please add notes here
